ELIGIBILITY REQUIREMENTS

Step 1. All currently employed, currently contributing self-employed or voluntary member.

• A). (One month loan) The member-borrower must have thirty six (36) posted monthly contributions,     six (6) of which should be within last twelve (12) months prior to the month of filing of application.
• B.) (Two month loan) The member-borrower must, have seventy two (72) posted monthly contributions, six (6) of which should be within the last twelve (12) months prior to the month of filing of application.

Step 2. The member-borrower whose employer must be updated in the payment contributions.

Step 3. The member-borrower has not been granted final benefit, i,e..., total permanent disability, retirement and death.

Step 4. The member-borrower must be under sixty-five (65) years of age at the time of application

Step 5. The member-borrower has not been disqualified due to fraud committed against the SSS.

LOAN APPLICATION REQUIREMENTS

Step 1. Member Borrower

• A.) Fill-up Member Loan Application Form
• B.) SSS digitized ID or E-^ ( Acknowledgement stub) With any two valid ID's one of which with recent photo.

Step 2. If filed by member borrower's Authorized Representative

• A.) Member borrower's duly accomplished member loan application.
• B.) Authorized Representative's SS card or any two valid ID's both with signature and at least one with recent photo.
• C.) Letter of Authority (LOA) signed by both the member borrower and the member borrower's authorized representative.
• D.) Member borrower's SS card or any valid ID's with signature and at least one with recent photo.

Step 3. If filed by Employer / Company's Authorized Representative

• A.) Member borrower's duly accomplished member loan application.
• B.) Authorized Company Representative (ACR) card issued by SSS
• C.) Letter of Authority (LOA) from employer and any two valid ID's with signature and at least with recent photo.
• D.) Member borrower's SS card or in its absence , Application for SS card (SS form E-6) acknowledgement stub and any two valid ID's both signature and at least one with recent photo.

Note: Original or Certified true copies of the supporting documents should be presented during the filing of the claim.

Where ? 

Located near LTO of Quezon City Compound ( kalayaan Avenue )

THE BASICS : As with any instance of needing to deal with government-type stuff, be sure to do the necessary preparation work before you even go there for processing your police clearance, here's what you need..

*** A black Pen

*** A photocopy of any valid ID with your Q.C. address on it.

*** Php 250 for fees

*** Wipes or Hand Sanitizer

Step 1. Present your original and photocopied valid ID at window one (1) you'll be given an application form. Fill out the form.

Step 2. After filling out the form, go directly to Window 2 secure your payment. request form.

Step 3. Pay the necessary fee Php 100 at the Treasury Office.

Step 4. After paying, go back to the Police clearance area, Have your fingerprints printed on your form.

Step 5. Proceed to the room where you would have to present your form and pay again.

Step 6. After paying, proceed to next step. Hand the staff your payment slip and application form, wait for your name to be called.

Step 7. Have your picture taken and signature. Look closely to your name, there might be corrections.

*** Exit the room and the be surprised by the people who will stare at you for a moment.

Step 8. Wait for your name to be called for police clearance and I.D.

Note::: Wrong information printed on your clearance and I.D.

Where you should get and apply for UMID Card?

** If you are a government employee go to the GSIS enrollment center.

** If you are private employee, self-employed or OFW you must be an SSS member so go to the SSS branch near you.

REQUIREMENTS AND PROCEDURE

Step 1. Bring the primary Valid ID's (Click here to view list of Valid IDs).

Step 2. Visit any of the following Philippine Government Agencies.

** Social Security System (SSS)

** Philhealth

** Government Service Insurance System (GSIS)

** Pag-ibig

Step 3. Get a UMID form.

** Form are available at enrollment centers.

** Accomplished the UMID form make sure to double check all the information you've intered.

Step 4. Verifying UMID form : Submit your form aling with your Valid ID's.

Step 5. ID Capture, Finger printing and signature. Once you've got verified you'll be asked to transfer 

to another window for and ID Picture. to  scan your fingerprints and to sign on signature pad.

Step 6. Wait for UMID Card to arive by mail : After your ID capture you will have to wait for 30 days to receive your UMID ID by mail if you fail to receive the ID after 30 days inquire to your nearest SSS or GSIS Branch office.

Learn How to get a Philippine Passport - Quick and Easy Way Tips

Step 1. Prepare your NSO Birth Certificate, you can call NSO Hotline (02) 737-111 for your Birth Certificate.

Step 2. Prepare three (3) Valid Id's (Driver's License, SSS ID, School I.D.) and other supporting documents indicating your full name, date birth and your home address.

Step 3. Go online and make an appointment with the DFA. Fill up the application form. download and print it out once you are done and always remember your appointment numbers.

Step 4. Make (2) Photocopies of each documents in your possession. (NSO) Birth Certificate, Valid I.D's application form and other supporting documents.

Step 5. Make sure to come on time on your appointment date with the DFA.

Step 6. Presents your Appointment form to the appointment counter.

Step 7. Get your queue number at the information counter and sit down at a given designated place while waiting for your turn.

Step 8. After processing you may proceed to the passport enrollment section at 2nd floor and pay the passport processing fee. (regular fee Php 950.00) 25 working days. (RUSH PROCEESING PHP 1200.00) 15 days and additional Php 200 will be charge for processing of lost passport if it is still valid.

Step 9. Next proceed to the encoding section for data capturing.

**** For passport renewal you will just need to photocopy the front and last page of your passport (Two Copies each) Also prepare a copy of your NSO Birth Certificate.

**** Meanwhile for lost passport you should execute and affidavit of loss. Prepare photocopies of your lost passport as well and have a copy of your NSO Birth Certificate and Other supporting documents.

How to get NBI Clearance

How to apply for NBI Clearance  Online - Step by Step Process

Here are the steps of how to Get and NBI Clearance through Online Application in the Philippines

COMPLETING THE APPLICATION FORM

Step 1. Visit the Website of the NBI clearance online application

(www.clearance.nbi.gov.ph)

Step 2. On the page, fill out the "First Timer or New Applicant' Application Form.

Step 3. After completion, you should tick the box for " I accept Term of service" and the "Sign Up" Button should be clicked. A window will appear, and you must press the " I agree " Button.

Step 4. You will be directed to another page where more details are required. You must continue to fill this page out and click " Save Information" button afterwards.

Step 5. You have already complete the NBI Application form. You can edit it by clicking the "Edit Information" Button which is at the upper right part of the screen.

Step 6. Once you are done with the application form, you will have to click "Application for clearance" you will go the next step. This button is at the upper right side.

Step 7. Then, a window will appear and you will be asked of the NBI Clearance Application Type that you need. You will Choose either New or Renewal Application from the drop down list.

Step 8. Next you need to write the type of valid ID that you will present during the process.

Step 9. After completing this stage, you have to click "I Agree". An Important message will appear, and you have to click "Okay" Button.

Step 10. Setting an Appointment Schedule.

SETTING AN APPOINTMENT SCHEDULE

After processing the online application form , you have to set a schedule of appointment for the processing of you NBI Clearance.

Step 1. Choose the NBI Branch where you will process the NBI Clearance. Select branch from the drop down list.

Step 2. You need to set the date and time for the process.

Step 3. After the scheduling go to the "purpose" of your application. choose between LOCAL or ABROAD.

Step 4. Next, You need to provide the "Purpose Detail. Do this selecting NBI transaction, you have to pick a method of payment. It can be through banks ( Online or over the counter), mobile (Using GCash) Bayad Centers, or you can go directly to the NBI office.

PROCESSING
